From A Medical Perspective, Who Are The Patients For Whom Implant Application Is Unsuitable?
- Patients with uncontrolled diabetes should be carefully evaluated as implant candidates. There is no rule that says diabetic patients cannot receive implants. It should be remembered that diabetic patients are not abandoned to their fate due to any illness. To provide a more evident example, significant surgeries such as cardiovascular surgery, orthopedic surgeries, and many others can be performed on diabetic patients. The important thing is to keep diabetes under control so that it does not negatively affect the healing process.
- We determine whether diabetic patients are suitable for implant treatment by looking at their hemoglobin A1C (HbA1C) levels in the blood. The hemoglobin A1C (HbA1C) levels indicate the patient’s average blood sugar (glucose) levels over the past three months, and based on these values, our clinical oral surgeons decide whether the patient is suitable for implant surgery.
- Another essential medical consideration is the patient's use of bisphosphonate drugs to increase bone density. When these drugs are used, implant placement is risky due to the excessive repair mechanism observed in the jawbones, and tooth extraction or prosthesis irritation can pose a risk. In such cases, candidates for implant surgery are evaluated based on their b-CTx levels in the blood. These marker levels in the blood provide expert doctors with an idea of whether the patient is a suitable candidate for oral surgical procedures.
- Another contraindication for implant surgery is an acute infection, tumor, or metastasis in the implant candidate area. Radiation therapy applied to the head and neck region for tumor treatment poses a separate risk for implant success. Particularly in cases where radiation therapy exceeds 60 grams, there is a risk of radiation-induced bone infection (osteoradionecrosis/ORN). Therefore, we recommend that patients who have received radiation therapy to the head and neck region wait at least 18 months before undergoing implant treatment. Allowing sufficient time after radiation therapy ensures safe implant placement for our patients.

When implants are placed in suitable anatomical areas within the bone, there are no pain receptors that would cause intraosseous pain, which means patients do not experience pain with local anesthesia. Therefore, implant placement is a painless procedure. Under local anesthesia, the patient does not feel any pain.
Additionally, with the painkillers and anti-inflammatory medications we provide to our patients, they do not experience post-operative pain, either immediately after implant surgery or within the 2-3 days following the procedure.
For our patients with sufficient bone volume, two visits are adequate. In the first visit, the implants are placed, and in the second visit, the permanent prosthetic structures (crown/bridge/hybrid prosthesis, etc.) are completed. In cases where the bone volume is insufficient, and implant placement is not possible, the procedure to increase the bone volume is performed during the first visit.

The healing process of dental implants is called osseointegration. In modern implantology, the osseointegration period takes around 2 to 4 months. While bone-implant integration is necessary for implant healing, there is also a separate healing process for the soft tissue around the implant, which involves the formation of healthy keratinized soft tissue. This process takes at least 21 days. To ensure the aesthetics of the crowns placed on the implant and the formation of healthy soft tissue, we recommend waiting at least 21 days before proceeding with permanent prosthetic structures.
- In cases of partial edentulism (single or multiple missing teeth), the preparation of natural teeth, which involves cutting and reducing the size of the teeth, is required for bridge construction. This process is called tooth preparation. Tooth preparation is irreversible and is not recommended for health reasons. In dental implant applications, tooth cutting or preparation is not necessary. Thus, missing teeth are compensated with dental implants, preserving the health of the natural teeth while allowing the patient to continue chewing, speaking, and maintaining aesthetics.
- Patients are freed from large and bulky prosthetic structures like removable dentures. The palate remains open with dental implants, and patients receive fixed prosthetic structures on implants replicating natural teeth.
- Aesthetics become much better and more satisfying.
- Speech function (phonation) continues much more efficiently with prosthetics supported by dental implants.
- Gag reflex is prevented with fixed prosthetic structures on implants.
Dental implants are more costly procedures compared to fixed prosthetics like bridges. However, the fact that dental implant procedures are more affordable in Turkey than in Europe and America eliminates this disadvantage. With this in mind, healthy teeth should not be prepared, meaning they should not be cut for bridges. This would lead to the loss of more teeth in the future. In the case of dental implant application, the lost teeth are regained, and since the teeth planned to be cut for a bridge will not be cut, those teeth can also be considered preserved with implant application.
